Vehicle owners pay an annual contribution fee, as established by the institution, each year this plate is renewed. It is paid as part of the registration renewal process. This fee is in addition to all other fees pertaining to the renewal of a vehicle registration that may apply and must be paid.
Yes, it may be personalized with up to five characters, if that combination of characters is not already issued, and the selection meets DMV criteria for issuance. See Form TC-817, "Application For Personalized Plates"

Utah collegiate plates are issued to vehicle owners who have made the required contribution to an academic scholarship fund at a participating institution of higher education. The donation can be paid at the DMV office or to the institution. The plates have the school logo on the left, with the school name or slogan on the bottom.
To obtain collegiate plates from your local motor vehicle office, you must make a minimum contribution to the institution, and provide the current registration certificate for the vehicle to which the collegiate plates will be assigned. You do not have to wait for your current registration to expire.
To keep the collegiate plates, an annual contribution must be made at the time of registration renewal to the same academic scholarship fund.
Minimum contribution amounts, established by individual institutions, are listed below. Each institution will accept amounts over the minimum level, but only one set of license plates per donation will be issued.
